Urethroplasty for poor patients.

 

Background, Scope and Impact

Urethral stricture & congenital urethral abnormalities (hypospadias) are more common among poor people. There are various temporary and permanent options for them. Urethroplasty is the permanent treatment option for them. But it is very much expensive. Most of them can’t bear the cost so they choose temporary options. Which insidiously make them cripple both physically and mentally . As well as it is costs like a slow poison. So a permanent operative procedure of urethroplasty is not only the cure of disease but also improve the quality of life , reduce the social burden of the patient. Our project aimed to help those poor patients. I’m as Urologist performing Urethroplasty operation for last ten years. Though it is very expensive(total cost may very from 20,000/- 1,50,000/ BD Taka according to severity of disease) I’m Trying to help those poor patients by donating their payment partially from poor fund (jakat fund). Donation amount – is about 20- 50 % of their total cost of operation. Which is about 10,000/- 30,000/ BD Taka per patient. (Average 20,000/ per patient).

Urology deals with male genitourinary system and female urinary system. There are many urological problems (both congenital and acquired) requiring reconstructive operation for the purpose of proper functioning of the genitourinary tract and maintenance of healthy life. The common problems are – Hypospadias (Abnormal urethral opening and abnormal genitalia) Urethral stricture (Abnormal narrowing of urethra) Posterior urethral valve (Abnormal membrane in the urethra causing outflow obstruction) Undescended testis (Abnormal position of testis) Vesico ureteric junction(VUJ) obstruction (Obstruction at the level of entrance of ureter into bladder Vesico ureteric reflux(VUR) (Abnormal retrograde flow of urine from bladder to ureter). Pelvi ureteric junction(PUJ) obstruction (Obstruction at the starting of ureter from the kidney) All these problems demand reconstructive operation and any negligence may lead to permanent impairment of genitourinary tract and even kidney failure. Underprivileged patients of those diseases may deprive from proper operative management due to lack of adequate awareness and economic restriction. Since all the reconstructive operations are very much expensive (cost may vary from 50,000- 1,50,000 BDT) we are trying to help these underprivileged patients by donating them from poor fund. According to my proposal each patient will bear only 10,000 BDT by themselves and rest (80-90%) expenditure will be donated from project.

Progress

During 2018/2019 a total of 8 patients were treated under HBB funding. The procedures in all cases were successful
